Course Information

Course Title: Digging Deep: Exercising Root Cause Techniques

Course Type: Workshops

Objective: This workshop provides information on root causes and contributing factors and reviews three techniques to conduct root cause analysis (RCA). This workshop evaluates a scenario with hands-on use of three techniques to identify root causes and contributing factors for an injury. The workshop concludes by using RCA results to identify corrective actions to improve the safety management system and prevent injuries and illnesses going forward.

Pre-Requisites: Individual DoD Components have training requirements for S&H professionals conducting mishap investigations. However, this intermediate-level workshop is beneficial to personnel scheduled to attend mishap investigation training, or those wanting to refresh their knowledge, or refine their skills, in RCA. Attendees should have a fundamental knowledge of RCA prior to the workshop. Personnel gaining the most benefit from this workshop may include: safety and health (S&H) professionals, supervisors, and other personnel involved in investigating incidents.

Lori has worked with the SMCX for more than 11 years, primarily working with the Air Force providing guidance for SMS implementation and sustainment. She has safety experience in the manufacturing industry, is an active volunteer with the ASSP, and has had several technical articles published.
